A conversation with the Office of Professions, who is processing my supervision-only work permit...
Me: Hey, so I'm calling to check in on the status of my permit.
Them: Okay. It looks like we got your check, and your license application, but not your Form 5 (the application for the supervision work permit itself). Do you know when you sent it in?
Me: Last Friday, but I have a certified mail receipt that says you guys got my application three days ago. Um... how is it that you can have the check, which I sent in together with the Form 5, and not the form itself...?
Them: Not sure, but try checking in next week to see whether the Form 5's been processed.
Me: Ah. Okay. Thanks for your help.
